If you or someone you know is 20 years old or younger and needs assistance obtaining medical/adaptive equipment or therapy services that is not covered by their insurance provider, please contact us via the form below so we can email you our Application for Assistance Form.
At this time, we are accepting applications from those living in Oregon, Washington, Idaho and California.
Please note: We do not pay deductibles or co-pays on insurance. At this time we do not purchase: iPads, iPhones, computers, e-bikes, hippotherapy, service animals, trexo robotics or vehicle and home modifications. If you are requesting a high priced item, if approved, we would commit to a certain amount of support and you will have to come up with the remainder of the cost. We only release funds to the vendor. We do not fund consecutive years. California, Idaho & Washington residents will be required to pay sales tax on the delivery of equipment.
